Case Manager

SailFuture case managers are passionate, understanding, and dynamic leaders who are committed to supporting and guiding teenagers through a tumultuous foster care system. 

Our specialized case management team serves the highest risk and hardest to reach teens in foster care in Pinellas County.

Our teens unmet behavioral health and mental health challenges manifest themselves through defiant and disruptive behaviors. 

SailFuture case managers recognize that these behaviors are a symptom of a systemic problem, not an indication of a child’s potential, worth, or character. Our responsibility is to interpret the message behind the outburst and help guide our teens to a place of stability and growth. 

With these challenges in mind, our case managers work with (10) youth at a time and provide wraparound services in addition to case management. 

By design, case managers are expected to build stronger and more supportive relationships with the youth under their supervision. Their work extends beyond traditional case management and is inclusive of academic, vocational, and emotional support.

A key part of our wraparound service model includes weekly sailing activities with the youth on your caseload, their caregivers, and their siblings. Our on the water experiences help us strengthen and accelerate relationships on land, while also providing a space and time outside of ‘foster care.’

Anyone applying for this position should possess a “whatever it takes” mentality when it comes to meeting the needs of the kids in their care.

This position will work in close partnership with the SailFuture school, mental health team, Group Homes, and the Life Skills Center.

This is a 45-hour per week salaried position and is not eligible for overtime pay.

Schedule:

M - F: 9am - 6pm

On-Call: One day per week, M-F (no weekend on call responsibility)

Responsibilities:

  • Complete home visits every 14-days
  • Respond to child’s immediate needs and provide daily de-escalation and conflict management 
  • Inspire and motivate youth to build life skills, including but not limited to employment, education, and extracurricular interests
  • Inspire and motivate parents to complete their case plan tasks
  • Document activities daily in Slack, Notion, FSFN, and other technology platforms
  • Write initial case plan if needed
  • Assign weekend plans for youth without permanent placement
  • Participate in on-call rotation (1 day / week)
  • Write judicial reviews
  • Submit regular judicial status reports
  • Submit paperwork in a timely fashion for social security and master trust funds
  • Procure and store vital records for each child (social security card, birth certificate, parents death certificates, etc.)
  • Procure passports, ID’s, and other forms of identification as needed
  • Work in partnership with the child’s GAL, state attorney, AAL, public defender, and other parties
  • Supervised visitations between child and parents
  • Provide referrals to parents as required by case plan and remove any potential barriers to parents accessing needed services
  • Maintain communication with parents’ service providers
  • Schedule and provide transportation to medical and dental appointments
  • Utilize personal vehicle to transport children (mileage reimbursement at .44c per mile)
  • Create and manage electronic case files for each youth in Google Drive
  • Conducting assessments and family integration and self-sufficiency plans 
  • Participate in weekly team meetings
  • Take 2 youth sailing once a week on your assigned sailing day
  • Participate in bi-monthly weekend sailing experiences
  • Being available and responsive to the child’s needs in times of crisis

Qualifications:

  • 4 year bachelor’s degree or higher
  • Clean and valid Florida Driver’s License and reliable transportation and proof of insurance (A Motor Vehicle Record will be requested from the DMV)
  • Child welfare certification (2-month course)
  • Team player committed to doing whatever it takes for youth in their care
  • Experience working with at-risk youth, specifically teenagers 
  • Excellent communicator and problem solver
  • Passion for adventure
  • De-escalation tactics (provided by SailFuture)
  • Behavior management (provided by SailFuture)

Compensation:

  • $39,000 annual salary
